How to Use Your Vision Benefits
Confirm Your Coverage
Call your insurance provider or log in to your member portal to check that your vision benefits are active and that District Vision Care is in-network for your plan.
Schedule Your Appointment
Call us at (801) 495-4833 to schedule your visit. Let us know your insurance provider when scheduling and we'll verify your benefits in advance.
Bring Your Insurance Card
Bring your vision insurance card to your appointment. We'll handle the billing directly with your insurer and explain any out-of-pocket costs before services are rendered.
We Handle the Rest
After your appointment, we submit claims directly to your insurance provider. You'll only pay any applicable copays, deductibles, or amounts not covered by your plan.
